Requalification Course Registration Info/Checklist – Class #75

You may not register for this course until you have submitted the Basic Academy Requalification Pre-Registration Form.

Do not complete any forms until we have collected your deposit. Submission of the following documents is required prior to the first day of class; failure to do so may result in cancellation of your reservation in the course. Please adhere to all noted time specifications.

Documentation:

  • After confirmed reservation the staff assistant will e-mail further college application instructions
  • Basic Academy REQUAL Cover Sheet
  • DOJ — take this form to a Livescan location (use a law enforcement facility to avoid possible delays in your clearance). Your clearance must be dated within six months prior to the start date of the class. We need the original document that the DOJ mails back to you after your fingerprints are submitted; NOT the receipt the Livescan operator hands to you on the day they take your prints. Be advised: The DOJ does not offer expedited services.  (Note: If you are coming from out of state to attend this course, let us know as soon as possible.  The course assistant will provide you with instructions on how to get this completed.)
  • Medical Exam Report — take this to your personal physician to complete no sooner than 30 days prior to the start date of your class.
  • Scan and email a clear copy of your valid CA driver’s license (or other government-issued photo ID)
  • Scan and email a clear copy of your medical insurance card (you must have coverage for the duration of the course)
  • Uniform: The class assistant will provide you with a link where you can order your Polo Shirt (2 Shirts are included in the fees).  Be sure to place your order at least a month before the class starts.  You may also visit our office to try on sample sizes before you order.  Refer to the Student Needs List below for additional uniform requirements.
  • Final payment must be paid in full at least five (5) business days prior to start date of your course

Agency Sponsored Students:

In lieu of completing the above mentioned paperwork, an agency clearance letter can be submitted to us on department letterhead with the following information

  • A Full-time employee
  • Valid CA Driver’s License
  • Firearms clearance from the CA Department of Justice
  • Successfully passed full physical examination
  • Employee maintains medical insurance through your agency
  • Will the recruits be using a department issued Firearm through the duration of the course? If so, please provide the Serial # and Caliber

Every recruit (including agency recruits bringing their own personal firearms) must submit proof of ownership for their firearm. Agency recruits supplied with agency firearms are exempt. If you have recently purchased a new firearm, bring in your receipt of ownership. Otherwise, you must complete the appropriate form, determined by your situation, found below. The form must be notarized and mailed, along with the required form fees, to DOJ. Email a copy of the signed and notarized form to us BEFORE MAILING ORIGINAL TO DOJ.

College Registration:

This course will be registered with one of our 7 Consortium Colleges. We request that you don’t apply to the registering College until instructed to do so. If you plan to apply for Financial Aid, including the BOG Waiver and the GI Bill, please inform the class assistant as soon as possible. Fee Advisory:  If you are not a current CA State resident -OR- if, on the start date of the course, you have not lived within the State of CA for one year and one day, you may be assessed out-of-state fees.
