- Download the State Fire Training Instructor Code of Conduct/Ethics
- Fill out the fillable form electronically (must have latest version of Adobe Reader) or print and fill out by hand
- If you filled out electronically, save the file, and upload it into the Fire Instructor Qualification Confirmation Form
*If you printed and filled out by hand, scan or take a photo of the form and upload to the Fire Instructor Qualification Confirmation Form.
