Student Checklist for EMT Course: 8/13/19 - 12/19/19

Requirements (must be met in order to register for this class)

*You must be 18 years of age at the start of midterms (dates may vary, contact our front office, 408-229-4299 for more information if you are under the age of 18)

Documentation:

You may not register for this course until you have submitted the Emergency Medical Technician Pre-Registration Form.

*Please note, the form will require you to upload the following documents:

  • A copy of your CA driver's license (or other gov't-issued photo ID)
  • A copy of the front and back of your medical insurance card (you must be covered by insurance throughout your class)
  • CPR training certificate (or statement verifying training equivalent to the current American Heart Association's Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care at the Healthcare Provider Level or BLS Level)
  • Test/vaccination record/results for:
    • Current Tuberculosis (TB) skin test (current=within six months prior to the start of class);
    • Current Tdap (current=within 10 years)*
    • Current MMR (Measles, Mumps & Rubella) vaccination (current=two doses in lifetime)*
    • Current Hepatitis B vaccination (current=three doses in lifetime or begin series prior to the start of class)
    • Current Varicella vaccination (current=two doses in lifetime)**
    • Flu Shot (for winter courses only; prior to start of class)

Once all of the above-listed items have been submitted and approved, a deposit will be collected to hold your spot in the class. Official acceptance into the course is contingent upon your final payment being received in full at least five days prior to the start date of the course.

Registration:

Contact our main office to register for this course. You will need to apply to Gavilan College through OpenCCC (this is a multi-step process detailed here); be sure to choose FALL Semester and submit the form found in the link as soon as you obtain both ID numbers.

Cancellation Policy:

There is a strict 30-day cancellation policy. Cancellations made after that time are subject to a $400.00 late cancellation fee.
