How do I sign up?


In order to sign up for any of the YALE programs, you and your parent(s) or legal guardian(s) must complete a set of forms and mail or fax them to our registration department.  Please mail your registration fee of $25.00 to the address below with a copy of your application.

What forms will I need?


You will need a YALE Application and a Liability Waiver.
